Event Registration Form

In order for us to make any event we run at Tara Springs as positive as possible for all participants, it is really important to us to have accurate information as to your current state of health and wellbeing.

Although constantly filling out forms is tedious, we like to see it as taking ownership of who and where you are at in life, and the more honest you are about exactly where you’re at, the more you stand to gain.

Emergency Contact Information

We need to know who to contact in the unlikely event that something may happen to you that requires support.

Your Health Status 

We require an honest self assessment of any circumstances regarding your health capacity, be it mental, emotional or physical  that would impact or interfere with your ability to undertake any activities in the event/class/workshop/retreat?

Please note that we advise participants to seek medical advice regarding any current or pre-existing medial conditions before attending the retreat. Modifications and variations can be provided if it is deemed suitable.

Consent to receive Touch 

Occasionally, touch and gentle alignment are offered as part of yoga events. Not everybody is comfortable with this, so we need to know where you stand on touch.

Dietary requirements 

Please let us know if you have any food allergies or specific dietary requirements.

While we will do our very best to accomodate them, we cannot guarantee our food is free from nuts, allergens.

Photography

On some of our events we take photos or videos for future marketing purposes, so we need to know where you stand on your images being used.
